As a key member of our team, you will play a pivotal role in implementing patient-specific care plans, administering medication, and ensuring the well-being of patients across various settings in our integrated network. Our approach focuses on a variety of facility-based and home-based patient environments, where your expertise and skills will contribute to our mission of enhancing patient outcomes and promoting recovery. Assess patient health problems and needs and develop and implement nursing care plans. Maintain accurate patient medical records and report any changes in patient status to the corresponding healthcare professionals. Administer medications, monitor patient recovery and progress, and educate patients and their families on post-discharge care. Oversee and direct nursing assistants and LPNs, ensuring the protocols and the quality of care met. Collaborate with multidisciplinary team members to enhance patient care and outcomes.
