We want your knowledge and expertise!Job Summary and QualificationsThe Diabetes Nurse Educator is a professional nurse, who independently directs, initiates and implements patient diabetes teaching and learning with a body of expertise knowledge and advanced skills, and ultimately inducing positive outcome to empower and promote behavior changes necessary for optimal health, psychological adaptation and quality of life for the patient with a chronic disease, Diabetes Mellitus. Utilizing a multidisciplinary approach, and working closely with nursing staff, physicians, dieticians, case managers, physical therapists, the diabetes nurse educator will assess, plan, implement, document, and evaluate educational instructions and outcomes given in a self-management directed program of diabetes education. What you will do in this role:
- Functions independently and collaboratively with hospital health care professionals and director in the interest and the achievement of optimal care for the patient with diabetes. -
- Focuses primary area of responsibility to the education of patients, their families and appropriate support systems about diabetes self management and related care issues. -
- Counsels patients and/or significant other(s) concerning self-management treatment responses to potential acute and chronic situations. -
- Utilizes the integration of a continuous nursing process beginning with a learning and training needs in-depth assessment, planning the teaching-learning process, and documents the education plan which includes but not limited to specific short and/or long-term behavior goals for diabetes self-management. -
- Documents patients and/or significant other(s) responses to education and progress toward achievement of educational objectives and behavior change goals. -
- Evaluates the outcomes of the diabetes education delivered to the patient with diabetes (with respect to plans of care and their implementation). -What qualifications you will need:
