HCA Healthcare is seeking a dedicated and compassionate Home Health RN to join our expansive team of healthcare professionals. In this role, you will provide direct patient care within the patient's home, implementing and documenting nursing interventions and coordinating overall nursing care for patients based on their individual health needs. With HCA Healthcare’s resources and support, our nurses have a unique opportunity to be advocates of patient care while maintaining autonomy and flexibility in their day-to-day work. Assess patient conditions during visits and carry out the appropriate care plans. Educate patients and their families on proper home health care strategies and procedures. Coordinate with physicians and other healthcare professionals to create customized care plans for each patient. Monitor patient recovery and compile reports for healthcare team making necessary adjustments to care plans. Ensure adherence to all health and safety standards and procedures. Support patient independence and treat them with dignity and respect.
