Assess patients' health problems and needs in their homes. Develop and implement nursing care plans for individual patients. Administer nursing care to ill, injured, convalescent, or disabled patients. Advise patients on health maintenance and disease prevention or provide case management. Monitor, record, and report symptoms or changes in patients' conditions. Maintain accurate patient health data and medical history. Collaborate with a team of healthcare professionals to coordinate patient care. Proficient in providing compassionate and efficient patient care. Able to assess and respond to patient needs quickly and accurately. Skilled in administering medication and operating medical equipment. Strong organizational skills and attention to detail.
