We are seeking a qualified Nurse Practitioner or Physician Assistant to join our team as a Primary Care Provider. This role involves working closely with patients to deliver continuous and coordinated care. The successful candidate will help in diagnosing and treating various conditions, managing overall patient care, and collaborating with other healthcare professionals to promote patient health. Conduct patient assessments, including physical exams and patient history evaluation. Develop and implement patient management plans, record progress notes, and maintain a comprehensive electronic medical record. Prescribe medications and treatments, and collaborate with healthcare teams to ensure optimal patient care. Advise patients on health maintenance and disease prevention. Facilitate referrals to other healthcare professionals as needed.
