Agape Care Group is seeking a dedicated Registered Nurse (RN) for our After-Hours Weekend Hospice team, delivering exceptional care across the Southeast. This role provides a unique opportunity to support patients and their families during off-hours, ensuring continuous, compassionate, and personalized care during weekends. Our RNs are pivotal in executing our mission of providing dignity and comfort, allowing patients to spend their final days surrounded by love and care. As a member of our team, you will work in various home environments and occasionally in facility-based settings, adapting to the distinct needs of each patient while upholding the highest standards of hospice care. Provide direct patient care in accordance with the established plan of care and hospice guidelines. Assess, plan, implement, and evaluate nursing care for hospice patients from admission to death. Work collaboratively with the hospice care team to coordinate and manage patient care at the end of life. Educate patients and their families on disease processes, end-of-life care, and comfort measures. Administer medications and treatments as prescribed by the physician. Document accurately and comprehensively in accordance with hospice regulations and company policy. Demonstrate effective communication and support during emergency situations.
