*We are Under New Ownership!**Oak Hollow of Sumter ...making a difference, one resident at a time.**Job Summary*We are seeking a dedicated and compassionate Speech Therapist to join our team. The ideal candidate will work with our residents to assess, diagnose, and treat speech and language disorders. This role requires a strong understanding of rehabilitation techniques and the ability to create individualized treatment plans that cater to the unique needs of our residents.The Speech Therapist will collaborate with families, and other healthcare professionals to ensure comprehensive resident care. Evaluates and treats residents and patients with temporary or long-term speech, hearing, swallowing, and cognitive disabilities in compliance with state and federal guidelines.*Duties** Conduct thorough resident assessments to identify speech, language, and communication disorders.* Develop and implement effective treatment plans tailored to each resident's specific needs.* Utilize evidence-based practices in speech therapy interventions.* Collaborate with other healthcare professionals to support the resident's progress.* Maintain accurate documentation of resident progress and treatment outcomes.* Stay updated on current research and best practices in speech therapy and resident care.* Screens and evaluates Resident and develops appropriate care plan.* Under a physician's direction, provides direct patient / resident treatment based on established plan of care.* Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.* Administers, scores, and interprets specialized hearing and speech tests.* Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.* Completes required forms and documents in accordance with company policy and state and/or federal regulations.* Maintains patient records in an accurate and timely manner.* Devises special tools and treatments for the specific needs of patients.* Other duties as assigned.*Experience** Proven experience in speech therapy, particularly in a long term care setting.* Strong knowledge of anatomy, physiology, and medical terminology related to speech and language development.* Familiarity with early intervention strategies and techniques.* Ability to perform patient assessments effectively.* Excellent communication skills, both verbal and written, for effective interaction with residents and their families.* Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident's protected health information.
