Job Description Summary Provides diabetes education to inpatients and/or outpatients [and family members as appropriate]. Supervisory Responsibilities: None Financial Responsibilities: None Essential functions: - Assesses the needs of patients referred for diabetes education.
- Develops a plan of care that prescribes interventions to attain expected outcomes.
- Implements and/or coordinates Diabetes Education.
- Provides instruction to patients and family members about life skills self-management; provides refresher education as needed.
- Provides instruction to staff about diabetes management issues.
- Evaluates the effectiveness of diabetes education toward attainment of outcomes.
- Promotes community outreach and participates in health screenings.
- Performs other miscellaneous and related duties as assigned or requested.
Physical Requirements: Physical requirements are primarily related to the essential functions of any job. In order to perform the essential functions of this position, you must be able to: - Stand, walk, sit, use hands, reach, stoop, talk, hear and smell.
- Lift or exert energy up to 25 pounds 50% of the time, 25 pounds 25% of the time and 50 pounds 25% of the time.
- Maintains close vision [clear vision at 20 inches or less], distant vision [clear vision at 20 feet or more], color vision [ability to identify and distinguish colors], peripheral vision [ability to observe an area that can be seen move up/down or left/right] and depth perception [three-dimensional vision, ability to judge distances]. Must possess the ability to: - read, analyze and interpret complex scientific, clinical or business journals, financial reports or legal documents.
