Description Description of duties: Participates in Intake Evaluations, Individual, Family, Collateral, and Group Therapy sessions; attends Case Conferences and Supervision sessions; documents sessions using the Electronic Health Record; maintains and updates client demographic information in client chart; evaluates client’s progress regularly; creates/updates Treatment Plans in collaboration with client & client’s family; maintains and keeps current all required consent forms.
