Job Description Primary Care Physician Opening Outside of PhiladelphiaOpportunity in Media, PAPhiladelphia 20mFull-Time | 5 Days/Week | Media, PennsylvaniaPosition Overview:Join a high-performing post-acute care team in Media, PA, where you will manage complex patient cases in a skilled nursing facility (SNF) setting. This role offers the opportunity to meaningfully impact the health and quality of life of vulnerable patient populations while working in a collaborative, interdisciplinary environment.Job Duties:• Provide primary care services for patients in skilled nursing facilities • Manage chronic and acute conditions while developing individualized care plans • Collaborate closely with interdisciplinary teams, facility staff, and clinical leadership • Lead care initiatives that support improved outcomes, patient safety, and care transitions • Maintain accurate and timely documentation in accordance with Medicare and regulatory guidelines • Serve as a clinical resource and support system within the post-acute setting Qualifications:• Minimum 1 year of clinical experience • Post-acute care experience helpful, but not required—comprehensive training and support provided • Active and unrestricted Pennsylvania medical license (or in process) • Active and unrestricted DEA/prescriptive authority (or in process) • Background in IM, FM, EM, hospital medicine, or related fields welcome • Strong commitment to high-quality, patient-centered care and leadership Compensation:• Fee-for-service (FFS) model • Expected annual earnings: $180,000–$291,000 • No cap on productivity income potential Position Highlights:• Flexible rounding schedule • Uncapped earning potential • Autonomy to structure your day while receiving robust organizational support More...
