Our focus is to promote healthy lifestyles, protect and nurture vulnerable children and youth and empower them to have a voice in their communities.SUMMARY:The Adoption Case Manager (ACM) will be responsible for assuring compliance with Adoption State and County regulations for children, youth and families for whom APM has received a Statewide Adoption and Permanency Network (SWAN) referral. The ACM will provide case management pertaining to adoption services to the children/youth, adoptive resource parent(s) and foster parent(s) in accordance with service description and Philadelphia Department of Human Services (DHS) contract requirements.Some duties and responsibilities include:Assessing the physical condition of the adoptive resource homeVisiting Adoption Finalization recipients, children and youth, once per month until finalization occurs to ensure that the needs of the adoption RP and child/youth are metCollecting and compiling child/youth information from various source and locations to provide to contractors for Child and Family Profiles, Child Preparations and FinalizationsMaintaining up-to-date files, according to state and county requirements, and provide status updates on families/servicesWorking in conjunction with a multidisciplinary team to provide SWAN servicesCompleting mandated reports, documentation and benchmarks in the time frames specified within both the Adoption and Foster Care contract requirementsAssisting with adoption training and social work services to adoptive familiesPreparing for and attending permanency conferences, child prep plan meetings, court, etc. as required WHAT'S IN IT FOR YOU?Health Insurance or $100 per month with proof of current Health InsuranceVision, Dental, and Life InsurancesRetirement Plan with company contributionFlexible Spending Accounts for health, child care, and public transportation expensesShort-term and long-term disability Employee Assistance Program including free counseling, trainings, webinars, and other resourcesMileage ReimbursementReferral ProgramLoan Forgiveness20 Days of Paid Time Off 12 Days of Paid HolidaysOther voluntary benefitsRequirements:EDUCATION: Completed Bachelor's Degree in Social Work, Psychology or related fieldEXPERIENCE: Minimum one year of experience working in the adoption system or other program with direct social service delivery (preferably in the Hispanic community).CLEARANCES: Pennsylvania Child Abuse Clearances, Pennsylvania Criminal Record Clearance, Federal Bureau Investigation (FBI), Medical ExaminationVALID DRIVER'S LICENSEAPM is an equal opportunity employer.
