Legacy Health, a prominent non-profit health system spanning Oregon and Washington, is seeking a dedicated Case Manager to join our dynamic team. This role entails comprehensive management of patient care, ensuring seamless communication and coordination between healthcare providers, patients, and their families. The Case Manager will play a pivotal role in enhancing patient outcomes, optimizing the utilization of healthcare resources, and supporting patients through their healthcare journeys across our hospitals and care facilities. Assess new patients, determine necessary care and services, and coordinate with healthcare professionals to set up patient care plans. Monitor patient progress and update care plans based on evolving needs and treatment results. Advocate for patients’ needs and rights for accessing necessary healthcare services. Facilitate multidisciplinary team meetings to ensure compliance with patient care plans. Educate patients and families about diseases, treatments, healthcare resources and discharge processes. Regularly document patient status and maintain comprehensive case management records.
