Description This position represents Providence Home Services (HS) at contracted facilities and requires strong communication and interpersonal skills. The role of the Clinical Liaison is to coordinate and arrange home care services between HS and contracted facilities for home health, home infusion, and home medical equipment associated with a clinical referral, except when there is an onsite HME liaison. • Oregon Registered Nurse License upon hire.
• National Provider BLS - American Heart Association upon hire.
• Upon hire: In compliance with the Providence Health and Services-OR Human Resources policy #302 Background Checks, a criminal background check is required for this position, based on assigned duties. Preferred Qualifications:
  • Bachelor's Degree - Nursing.
  • 1 year - Full-time experience in home health or hospice.
  • 1 year - Full-time experience in home infusion.
  • 1 year - Full-time experience in discharge planning or care management.
