Position Summary:
To provide individualized psychotherapy, family education, and therapeutic group sessions to acute inpatient gero-psych patients aged 60 and over. - Supports discharge planning and utilization review services within a multidisciplinary treatment team in the Senior Behavioral Health Unit.
Minimum Qualifications:
- Master of Social Work (MSW) degree required, License Certified Social Worker (LCSW) in the state of Oregon preferred.
- Minimum 1-year internship experience, preferably in a hospital or health care setting.
- Must have the ability to work efficiently, responsibly, and independently.
- Excellent time management skills required.
- Must possess strong interpersonal skills with a high sensitivity to the needs of others, including the ability to communicate effectively with staff, administrators, patients, families, physicians, volunteers, and other customers.
- Nonviolent Crisis Intervention Training within ninety days of hire.
Essential Job Functions:
- Complete Psychosocial Assessment, Mental Status Exam, and Alcohol Use Disorders. Identification Test within 72 hours of admission.
- Coordinate and contribute towards individual and group therapy activities according to an established program schedule.
- Coordinate discharge planning in conjunction with the multidisciplinary team.
- Offer family education as needed to complete discharge coordination.
- Participate in daily treatment team.
- Complete utilization review in an organized and timely manner.
- Knowledge of Community Resources and strong networking
- Assumes responsibility for meeting annual in-service requirements and ongoing personal development
