This role requires a robust understanding of healthcare procedures and patient support, ensuring the development of care programs that enhance patient outcomes and satisfaction. Evaluate patient needs and develop comprehensive care plans in collaboration with healthcare providers. Facilitate multidisciplinary team meetings to ensure seamless communication and cohesive patient strategies. Monitor patient progress and adjust care plans as necessary. Provide patients and families with understanding of health conditions, medical options, and resources. Ensure compliance with healthcare standards and ethics in care management.
