Advanced Patient Care Pharmacist - Co-Director of Antimicrobial Stewardship

The Advanced Patient Care Pharmacist is responsible for working with the multidisciplinary patient care team which may include prescribers, nurses, pharmacists, dieticians, and other members of the patient care team (i.e., laboratory technicians, respiratory therapy) to provide optimal pharmaceutical care to Nationwide Children's Hospital patients in a focused practice area. The Advanced Patient Care Pharmacist demonstrates competency across all areas of pediatric clinical pharmacy practice, and provides age-appropriate care to patients from the age of birth through 21 years, and select adult patient populations. In addition, the Advanced Patient Care Pharmacist routinely participates in teaching, research, quality improvement endeavors, and scholarly activities.

Responsibilities

  • Serves as a member of the patient care team as the pharmacotherapy expert, such as: rounds with the medical team to prospectively recommend and evaluate individualized drug therapy, makes recommendations to achieve best outcomes in an evidence-based and patient-specific manner while considering pharmacoeconomic principles, and prospectively identifies potential adverse drug reactions, drug-drug, drug-disease and drug-food interactions, and incompatibilities affecting patient outcomes.
  • Coordinates and maintains the antimicrobial stewardship program, including:
  • Overseeing daily stewardship operations
  • Create agile and innovative programs to improve antimicrobial utilization throughout the organization
  • Maintain antimicrobial formulary and work with infectious diseases service line to evaluate appropriate use of restricted antimicrobials
  • Develop pathways for appropriate antimicrobial use in collaboration with the infectious disease team
  • Monitor and trend antimicrobial usage throughout the Nationwide Children's Hospital organization, including inpatient, ambulatory and accountable care areas
  • Documents direct patient care, clinical, operational, and financial outcomes and activities appropriately in the medical record.
  • Provides appropriate handoff for pharmacists and other healthcare providers to follow up on anticipated pharmacotherapy needs, including medication changes. Collaborates with other members of the pharmacy team to assure continuity of care and consistency of practice
  • Participates in the clinical pharmacy on-call program, including weekend coverage.
  • Engagement in annual Professional Development Index expectations, including:
  • Precepts PGY2 Pediatric Pharmacy Residents, PGY1 Pharmacy Residents, Doctor of Pharmacy students and Pediatric Pharmacy Fellows
  • Provides team-based education and training to medical staff, fellows, residents and students. Develops, provides and participates in Pharmacy Department staff development activities
  • Participates in research and scholarly activities as an investigator or consultant to the pharmacy, medical, and nursing staff. Routine contribution and external scholarship is expected through publication of articles in peer-reviewed journals or conference abstracts
  • Participates on departmental and/or hospital committees as related to medication safety and/or quality improvement initiatives while demonstrating pharmacy practice leadership. Minimum involvement in one or more committee is expected in addition to routine medication safety contributions.
    Qualifications

    KNOWLEDGE , SKILLS AND ABILITIES REQUIRED

    Education:

    * Doctor of Pharmacy (preferred) or Bachelor's Degree from an accredited college of pharmacy.

    Licensure:

    * Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio

    Certification:

    * BPS Board Certification (or eligible for testing, with certification to be obtained within 24 months of hire)

    Experience:

    * Minimum qualifications: PGY1 and PGY2 pharmacy residencies (or PGY1 residency and PGY2 fellowship) OR PGY1 residency with 3 years of hospital pharmacist practice experience (pediatrics preferred) with responsibility for direct clinical patient care similar to that provided through a PGY2 residency, OR 5 years of hospital pharmacist practice experience (pediatrics preferred) with responsibility for direct clinical patient care similar to that provided through a PGY2 residency
