As a Registered Nurse in our Hospice Home Care team in Franklin, you will be at the forefront of our effort to provide holistic end-of-life care. This role requires a dedicated professional who can administer hands-on nursing care while also providing emotional support to patients and their families. Provide direct patient care in accordance with home care policies Work closely with patients and their families to provide a full continuum of care Administer medications, document patient care services and manage symptoms Develop and revise care plans based on patient needs and doctors’ orders Coordinate with healthcare providers to palliate patients' end-of-life symptoms Educate patients and families on treatment expectations and end-of-life preparations Proficient in using electronic medical records Ability to work autonomously in a home setting Strong crisis management and decision-making skills Effective communication and compassionate interpersonal skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Registered Nurse (RN) license Certification in Hospice and Palliative Nursing (CHPN) preferred More...
