Transitions LifeCare, a leader in hospice, and palliative care, is hiring immediately for a Weekend On-Call LPN Part-Time position. As part of our compassionate team, you will play a vital role in providing exceptional care and support to patients navigating end-of-life or life-changing illness.This position offers a sign-on bonus and is ideal for those looking to contribute to a dynamic healthcare team while providing flexible weekend hours.The schedule for this position is Saturday/Sunday 9:00am-9:00pm (hours can be flexible).Responsibilities:Patient Follow-Up: Return or make follow-up phone calls to patients as directed by the Triage RN.Assessment & Recommendations: Under the guidance of the Triage RN, make recommendations related to patient assessments, ensuring adherence to policies and procedures.Collaborate with Physicians: Communicate any changes in patient status or needs with attending physicians or TL on-call physicians.Patient & Caregiver Education: Instruct patients and caregivers on any changes to the patients Plan of Care.Coordinate Additional Care: Assign visits to RN, Social Worker, or Chaplain when necessary, in collaboration with the Triage RN.Requirements:Licensure: Current LPN license in North Carolina (BSN preferred).Experience: Previous experience in hospice, home health, or palliative care is a plus.Transportation: Must have reliable transportation to visit patients in the service area.Compliance: Must pass all pre-employment and annual tests and meet state and federal guidelines.What You Can Expect from Us:Quick Communication: Our hiring team will contact you within 1-3 business days after your application submission.Interview Process: Participate in 1-2 interviews with the hiring manager, either virtually or in-person.New-Hire Process: Review job description, complete new-hire paperwork via UKG, and attend orientation and clinical skills training.Ongoing Support: Continuous training and professional development opportunities to help you provide exceptional care to our patients.Essential Functions:To perform this job successfully, an individual must be able to perform each essential duty.
