In this vital role, you will collaborate with a multidisciplinary team to support patient treatment plans, ensuring the delivery of personalized care that aligns with our commitment to help patients achieve their highest levels of health and independence. Provide one-on-one nursing care to patients in their homes, following prescribed plans of treatment. Monitor patient health status and respond to changes in condition. Administer medications, conduct routine assessments, and document patient progress. Educate patients and their families on treatment plans and in-home health management. Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care. Maintain patient confidentiality and adhere to HIPAA regulations. Strong clinical skills in a home health setting. Ability to work independently with minimal supervision. Excellent communication and interpersonal skills. Proficient in using medical software and electronic health records.
