In this vital role, you will manage medication therapy and oversee the pharmacy services across our inpatient rehabilitation hospitals, home health agencies, and hospice agencies. Our ideal candidate is a meticulous and compassionate healthcare professional adept at ensuring safe and effective patient care by providing optimal medication management, counseling, and education to both patients and healthcare staff. Provide accurate preparation and dispensing of medications according to prescriptions. Collaborate with healthcare team members to formulate, implement, and monitor individual patient medication plans. Ensure compliance with federal, state, and local regulatory requirements. Educate patients and healthcare professionals on medication use, side effects, and interactions. Manage inventory and procurement of medications and pharmacy supplies. Maintain accurate patient medication records and pharmacy documentation. Participate in quality assurance and performance improvement initiatives.
