Our focus is on delivering personalized, patient-centered pharmaceutical care in inpatient rehabilitation settings as well as through home health and hospice services, guaranteeing a dynamic and fulfilling work environment. Manage medication therapy and patient medication counseling. Review prescriptions to ensure accuracy and suitability of prescribed medications. Collaborate with healthcare team members to strategize and optimize patient treatment plans. Ensure compliance with federal, state, and local regulations and guidelines. Maintain accurate records of medication dispensing and patient care. Educate patients and caregivers on medication usage and side effects. Supervise pharmacy technicians and interns to ensure efficient workflow and compliance with standards. Strong knowledge of pharmacology and pharmaceuticals. Ability to work in a dynamic environment across different care settings. Excellent communication and interpersonal skills. Detail-oriented with strong organizational skills.
