Our MDS Coordinators become family and impact the lives of our residents who live in a Lifespark Skilled Nursing Community.In this position you will be responsible for overseeing and facilitating the successful scheduling, completion, and submission of all necessary documentation (MDS) to ensure compliance and positive resident outcomes. The MDS Coordinator will monitor coding and documentation to ensure accuracy and conducts and coordinates ongoing resident assessments necessary for the accurate completion of the Minimum Data Set (MDS), Care Area Assessments (CAA), and the residents Plan of Care.
