Job Description PDTXperts is seeking a Skilled Nursing Facility (SNF) Physical Therapist for a job in North Reading, Massachusetts. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Duration: Ongoing - 40 hours per week - Shift: 8 hours, days - Employment Type: Staff Requirement: Graduate of an American Physical Therapy Associations accredited Physical Therapy degree program Experience and Licensing Requirements: State Physical Therapy License - state of MA Current CPR certification. Minimum 1 year working in SNF setting and experience in Net Health Authorization to work in the United States. Demonstrated ability to work independently and collaboratively in a fast-paced environment. Job responsibilities: Conduct thorough assessments to evaluate patients' physical and functional status. Develop individualized treatment plans based on assessment findings and patients' goals. Implement evidence-based therapeutic interventions to promote mobility, strength, and function. Monitor patients' progress and modify treatment plans as necessary to achieve desired outcomes. Educate patients and their families on exercises, techniques, and strategies for self-management and injury prevention. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans. Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and ensure continuity of services.
