Encompass Health is seeking a dedicated Case Manager to join our innovative healthcare team. This role requires a compassionate and knowledgeable professional to oversee the integrated care plans of patients in our facilities spanning 42 states. The successful candidate will be responsible for coordinating with healthcare providers, patients, and families to ensure high-quality, patient-centered care following our model of facility-based and home-based services. Through effective case management, the Case Manager will facilitate individualized care that promotes recovery, health, and well-being.
