MedStar Health, a leading not-for-profit healthcare organization, is seeking a skilled RN Inpt Case Mgr to join our team. In this role, you will facilitate an integrated and coordinated approach to patient care, ensuring that patients receive comprehensive and timely treatment plans tailored to their medical needs. Working within our network of hospitals and care facilities in the D.C.-Maryland-Virginia region, you will help uphold our commitment to a patient-first approach by leveraging your nursing expertise and case management skills to positively impact patient outcomes. Conduct comprehensive patient assessments to identify healthcare needs and goals. Develop, implement, and regularly update individualized patient care plans. Coordinate care with physicians, nurses, and other healthcare professionals to ensure timely provision of services. Educate patients and their families about health conditions and treatment plans. Monitor patient progress and adapt care plans as necessary. Ensure compliance with healthcare regulations and maintain documentation of patient care. Advocate for patients' needs and act as a liaison between patients and healthcare providers. Strong clinical skills in patient assessment. Excellent communication and interpersonal skills for interacting with patients, families, and medical staff. Ability to manage multiple cases efficiently without compromising care quality. Experience in utilizing electronic health record (EHR) systems. Critical thinking and problem-solving skills. Medical Insurance Life Insurance Registered Nurse (RN) license in the state of Maryland, D.C., or Virginia. Certification in Case Management (CCM) preferred but not required. More...
