In this role, you will be integral to providing top-tier pharmaceutical care in accordance with our commitment to patient-first health services. Our Clinical Pharmacists are crucial in the direct management of complex medication regimens, educating patients on best practices, and overseeing the use of medications across the organization. Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability to ensure effective patient therapy. Advise healthcare team on the selection and effects of drugs, including drug interactions and side effects. Manage and monitor patient medication therapy and provide medication therapy management. Educate patients on medication usage and lifestyle changes for enhancing their medication therapy management. Implement, monitor, assess, and adjust patient-specific pharmacotherapy plans. Collaborate with pharmacy staff to optimize pharmacy operations and patient care. Strong knowledge of pharmacotherapy and pharmaceutical care principles. Ability to analyze complex medical data. Proficient in utilizing electronic health record systems. Excellent communication and counseling skills. Robust organizational and time management skills.
