Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) is required

  Position Summary
  
  The Home Care Social Worker supports MaineHealth Care at Home's (MHCAH) mission, vision, and values by exhibiting the following behaviors: excellence, competence, collaboration, innovation, respect, patient and family centered care, commitment to our community, and accountability. The Social Worker provides skilled, cost effective social work care in a competent holistic manner for a designated group of patients, through assessment, planning, implementation and evaluation of social work care, to empower patients and families to achieve maximum levels of wellness and independence. This position reports to the Clinical Manager.
  
  Required Minimum Knowledge, Skills, and Abilities (KSAs)
  
  
  
• Education: Graduate of a Master's Program in Social Work with one of the following:
  
  
  
  
  
  • LCSW certification requires two years of professional level experience.
    
  • LMSW-CC certification requires one year of professional level experience.
    
    
    
    
  • License/Certifications: See "Education." State of Maine licensure as an LCSW or LMSW-CC. Current, valid driver's license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy. Current certification in Basic Life Support for Healthcare providers.
