Assess, plan, implement, and evaluate patient care in a Long-Term Acute Care Facility. Collaborate with physicians and multidisciplinary team members. Administer medications, maintain accurate patient records, and ensure compliance with health and safety standards. Provide education and support to patients’ families about long-term care processes. Monitor patient recovery and responding to emergencies appropriately.
