License: State License required. Certifications: BLS required. Must-Have: Mandatory Reporter certification required. Profile must include Driver's License. Work history/resume required. Skills checklist must have been completed within 1 year of submission date. License verification required. Copy of required certifications. One supervisory reference required at the time of submission. Must have a taxable address outside of 50 miles from the facility.
