Job Description OpTech is a great employment opportunity for a Hospital-Based Pediatric -Social Worker (LMSW/LLMSW) Location: Detroit, MI, onsite Compensation: Up to $57/hour! OVERVIEW provides direct support to pediatric and adolescent students and their families, addressing social, emotional, and environmental needs that impact overall well-being and academic success. This role partners closely with students, families, and school staff to develop and implement individualized care plans in both school and home care settings. Key Responsibilities: Provide social work services to pediatric and adolescent students using the full social work process (assessment, planning, intervention, and evaluation) Collaborate with school staff, families, and interdisciplinary teams to support student success and well-being Coordinate care to ensure effective outcomes, quality service delivery, and appropriate use of resources Assist with discharge planning and transitions of care as needed Connect students and families to community resources and support services Maintain clear, consistent communication with students, families, and school personnel to promote engagement and satisfaction Adapt quickly in a fast-paced environment with minimal on-site training Required Experience: 5 years of case management experience 2 years of recent acute inpatient case management experience Prior discharge planning experience Experience working with pediatric and/or adolescent populations Previous home care experience preferred Valid Michigan driver’s license and reliable, insured transportation Education/Certifications: Master’s Degree in Social Work (MSW) from an accredited program Active Michigan Social Work license (LMSW or LLMSW) OpTech is an equal opportunity employer and is committed to creating a diverse environment.
