Pay Information $2,240 per week About The Position Job Title: RN - Home Health Profession: Nursing Specialty: Home Health RN Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 1 year in Acute Care Nursing; Preferred 1 year in Home Health Nursing License: Current RN License Certifications: BLS Required; Valid Driver's license and proof of personal auto insurance Required Must-Have: Strong computer skills with the ability to navigate EMR programs Excellent communication and customer service skills Ability to work independently, prioritize and complete tasks in a timely manner Description: The RN is responsible for assessing, planning, coordinating, implementing, and documenting care for an assigned caseload of patients in the Home Health department. Develops and implements a home health plan that meets each patient's specific needs and complies with regulatory agencies, federal and state regulations, and agency policies and procedures. Performs accurate and complete assessments of patients and conducts initial admission assessments. Coordinates and ensures the completion of interdisciplinary screenings and assessments. Assesses and reassesses pain; ensures assessments are appropriate for age, culture, and psychosocial factors. Recommends appropriate referrals to other disciplines and community resources. Formulates a plan of care based on patient needs and modifies the plan based on patient outcomes. Develops a teaching plan according to identified learning needs and evaluates its effectiveness. Educates the patient and family regarding treatment plans and provides discharge planning/instructions. Makes comprehensive nursing decisions based on the interpretation of facts and evaluation of patient outcomes. Participates in care conferences to discuss multidisciplinary team responsibilities, patient progress, and plans for continued care. Utilizes time management skills to complete care for a full load of patients within scheduled work hours. Communicates with physicians to obtain, renew, and update orders to reflect changes in patient condition. Prepares and administers medications based on age and clinical condition of the patient. Starts intravenous lines for medications and fluids; maintains PICC and Central parenteral lines per protocol. Assesses the effectiveness of pain medications and revises the plan of care according to the patient's response to treatment. Submits updated nursing care plans for weekend/holiday RNs. Maintains personal safety while providing care. Ensures documentation is complete, accurate, and timely; communicates documented information for evaluation. Completes daily visit sheets. Performs all aspects of the position in an environment that optimizes patient safety and reduces the likelihood of healthcare errors. Utilizes established hand-off communication protocols. Complies with all established safety procedures to ensure a safe environment for patients, visitors, and staff. Participates in performance improvement activities.
