Qualified Intellectual Disabilities Professional (QIDP) - Springfield, ILSUMMARYUnder the supervision of the Director of Outreach and Case Coordination, is responsible for all duties related to the development, implementation, and coordination of program plans for the children served within the assigned home(s). WHAT YOU WILL DOFunctions as a leader of the child's interdisciplinary team (I.D.T.), instructing Hope Residential Academy staff in the implementation of the children's daily schedules and behavior intervention techniques.Coordinates, develops, implements, and maintains individual program plans for each child within the Plex assignment, including the child's permanent record, all quarterly and annual reports (narratives, graph's etc.), and parent/guardian staffing.Communicates regularly with parents, guardians, and other resource persons/consultants regarding the child's care, issues, and program implementation and acts as liaison with community service providers and agencies.Reviews each child's progress at least monthly, and revises or replaces programs as necessary.Keeps all essential paperwork up to date which includes but is not limited to the ISP, ICAP, Assessments, monthly progress notes, plex meeting notes, and monthly calendars.Attends and prepares reports for DCFS which includes the youth's Annual Case Review (ACR) along with monthly and/or quarterly meetings.Serves as a member of daily rounds, residential review, referral review, best placement, attends screenings, behavior intervention committee, behavior treatment committee, feeding team, behavior clinic and other pertinent meetings.WHAT YOU WILL BRINGBachelor's degree in a Human Services Field (consideration may be provided to bachelor level degrees with sufficient course work [> 30 semester hours] in human services areas).At least one year of experience working directly with people with intellectual disabilities or other developmental disabilities.Ability to qualify as a QIDP with the Department of Human Services and maintain that qualification. Navigational mastery of various software systems such as Microsoft Outlook, ADP, Catalyst and Therap.Ability to establish, develop and maintain cooperative relationships with co-workers, families, and staff while working professionally with people from a wide variety of cultural and socio-economic backgrounds.WHAT WE PREFERMaster's degree in a Human Services related field3-5 years' experience as a QIDP or support role with children's group homes.
