Job Description:

MDS Coordinator is responsible for documentation verification of all nursing care administered. MDS Registered Nurse Coordinator is responsible for resident MDS data collection and documentation, completion of resident assessments, care plan, Rap summaries. MDS Coordinator complies with OBRA requirements, complete care plan schedules and meet with families and residents to discuss plans of care. MDS Coordinator collaborates with the interdisciplinary team to discuss resident plans of care.

Requirements:

* Must have Registered Nurse RN license in good standing in Illinois
* Minimum 3 years relevant experience
* Previous MDS / Long term care experience preferred
* Familiar with PDPM requirements
* Demonstrates caring and compassionate nature
* Must possess excellent supervisory skills
* Must possess strong computer skills
* Candidate must be a team player, have a positive outlook and excellent problem solving skills
* History of excellent attendance
