Job Summary and Qualifications -This position is responsible for the coordination of inpatient diabetes education throughout the hospital and/or outpatient diabetes education in the Diabetes Center located at the Wellness Center. - - What you will do in this role: -Conducts needs assessment, plans, implements, and evaluates diabetes education programs for patients, families, and staff. Assesses the patient's health literacy and utilizes appropriate level of language by using clear, simple terms and explanations. -Develops an individualized plan of care for each patient involving the family and based on current diabetes care standards. Incorporates cultural/spiritual concerns of the patient into the plan of care. - Utilizes age-appropriate learning and behavior change goals with clients.
- Recommends basic lifestyle changes and makes referrals and other interventions as appropriate
- Monitors outcomes of medical and nutrition therapy and recommends therapy revisions as needed
- Negotiates a discharge or post-program plan with clients
- Participates in patient and public education programs about diabetes
- Provides professional expertise to other members of the diabetes care team and the community
- Plans and implements periodic public education programs about diabetes and participates in health fairs. - -
