Registered Nurse (RN) | Case Manager Location: Kennesaw, GA Agency: Lancesoft Inc Pay: $2,688 per week Shift Information: Rotating - 3 days x 12 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Key Responsibilities (Summary) Coordinate patient care plans across departments. Conduct assessments and manage patient transitions throughout the care continuum. Collaborate with physicians, nursing staff, and support teams to ensure efficient and effective care. Monitor patient progress and support safe discharge planning. Maintain documentation and follow regulatory/organizational standards.
