As a Registered Nurse / Patient Care Manager at AccentCare, you will be entrusted with the provision of advanced in-home nursing and patient management. This pivotal role involves both direct patient care and administrative responsibilities, including the supervision of nursing staff and oversight of patient care plans. Conduct comprehensive patient assessments to determine the appropriate care plan. Oversee and manage a team of nursing and healthcare professionals. Coordinate with physicians and other healthcare providers to update patient care plans. Provide direct care and perform medical procedures as necessary. Maintain accurate documentation of care provided and monitor compliance with health and safety regulations. Educate patients and their families on disease processes, medical procedures, and other pertinent health matters. Administer medications and monitor responses to treatment. Strong leadership and team management skills Excellent communication and interpersonal skills Proficient in electronic health records (EHR) systems Adept at multitasking and prioritizing in a dynamic environment In-depth knowledge of healthcare protocols and best practices Medical Insurance Dental Insurance Vision Insurance 401k Valid RN license in the state of practice CPR certification Additional certification in home health care nursing is preferred.
