Job Description Job Description A-Line Staffing is now hiring a RN Case Manager. This will be full time. RN Case Manager Hours M - F 8am to 5pm On call 1 weekend/month and occasional call 1 day after hrs call during the week days County Covered: Jackson and Barrow Counties RN Case Manager Compensation The pay for this position is $45.00-48.00 an hour Benefits are available to full-time employees A 401(k) with a company match is available for full-time employees. RN Case Manager Responsibilities Perform comprehensive assessment of high risk members. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the member. Address, identify, and continuously reassess cost-efficient, appropriate levels of care. Put non medical support in place to ensure compliance with treatment plans (such as housing and transportation). Coordinate care transitions as needed. Coordinate with physicians, social workers, discharge planners, and claims professionals in transitioning patient to appropriate level of care. Engage specialty resources as needed to achieve optimal resolution. Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention. Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes. RN Case Manager Requirements RN License Oasis assessment, Wound care, neg pressure device mgt, PICC and Central line MGT, foley insertion and mgt, pleurX drain mgt, ostomy care and mgt, trach care, and LVAD mgt.
