Provide direct nursing care according to the prescribed patient care plan. Monitor patient’s condition and assess their needs to provide the best possible care. Administer medications, maintain accurate patient records, and report any changes in patient’s conditions. Assist with activities of daily living such as personal hygiene, meal preparation, and mobility. Collaborate with registered nurses and physicians to create a seamless environment of care. Educate patients and their families on proper home health care strategies and procedures. Proficiency in administering medications and operating medical equipment. Strong observational and nursing diagnosis skills. Effective communication and interpersonal skills with patients, families, and medical teams.
