Monitor and record patient's condition and document provided care via the electronic health record. Administer treatments and medications as prescribed by the attending healthcare provider. Communicate with multidisciplinary teams to facilitate comprehensive patient care. Educate patients and their families on treatment plans and ongoing needs post-hospitalization. Implement patient care plans and assist in the monitoring of complex health issues.
