The ideal candidate will manage patient cases across our inpatient rehabilitation hospitals, home health agencies, and hospice agencies, ensuring the highest standards of care are maintained throughout the patient's treatment journey. Assess new patients, identifying their needs and eligibility for specific healthcare services. Develop, implement, and review treatment plans in collaboration with healthcare professionals. Facilitate communication between patients, family members, and healthcare providers to ensure clarity and consistency in the treatment process. Monitor patient progress and make necessary adjustments to the treatment plan. Ensure compliance with all healthcare regulations and maintaining accurate patient records. Coordinate resources and services to meet the needs of patients efficiently. Educate patients and families about health status, treatment plans, and potential outcomes. Strong knowledge of healthcare systems and case management principles. Ability to solve problems and make decisions based on patient needs and care standards. Excellent communication and interpersonal skills for interacting with patients, families, and an interdisciplinary healthcare team.
