In this role, you will be pivotal in linking our inpatient rehabilitation, home health, and hospice services to deliver seamless care that promotes patient well-being. You will collaborate with multidisciplinary teams to assess patient needs, plan and coordinate continuing care, and ensure compliance with healthcare regulations while striving for the highest standards of care. Assess patient needs and develop individualized care plans in collaboration with a multidisciplinary healthcare team. Coordinate all phases of care from admission to discharge and follow-up. Act as a liaison between patients, families, healthcare teams, and external partners to ensure comprehensive care delivery. Monitor patient progress and adjust care plans as needed. Ensure compliance with all healthcare regulations and maintain up-to-date documentation. Provide education and resources to patients and families to support informed decision-making and improved health outcomes. Participate in quality improvement initiatives to enhance service efficiency and patient satisfaction.
