This role is crucial in providing tailored nutritional care that aligns with our commitment to high-quality and cost-effective treatment. You will be directly involved in the dietary management of patients across our facilities and home-based settings, ensuring that each patient’s nutritional needs are met according to the latest clinical guidelines. Assess nutritional needs, diet restrictions and current health plans to develop and implement dietary-care plans and provide nutritional counseling. Collaborate with a multidisciplinary team to provide comprehensive patient care. Advocate for and educate patients and healthcare team members on the importance of nutrition in preventative care and disease management. Monitor and evaluate patient progress toward desired health outcomes, revising dietary plans and goals as needed. Stay current with the latest nutritional science research and integrate evidence-based practices into patient care. Prepare reports and maintain accurate patient records in compliance with HIPAA.
