Office requirements are minimal and limited, but can be adapted based on your work preference. This position has a flexible schedule Monday Friday, with no on-call hours or weekend hours required. Requires an ability to work independently and maintain work requirements within required time frames.Case managers work with a wide variety of individuals who may be diagnosed with a mental health, substance use, and/or co-occurring disorder using a strengths based approach. Case Managers may also work with individuals who may have/had criminal justice involvement, high utilization of services, pregnant woman or women with dependent children. Efforts are made to effectively link them to needed community resources, coordinate such resources as necessary, and monitor the implementation of such. Examples of such resources may include, but are not limited to, psychiatric services, substance abuse services, rehabilitation services, Supplemental Security Income (SSI), Medicaid, Patient Assistance Programs, employment, assistance with legal Issues, transportation and social supports.We offer mileage reimbursement for all community travel done on behalf of a patient.
