Position Overview:We are seeking a compassionate Physical Therapist Assistant (PTA) to join our team at a Skilled Nursing Facility (SNF) in Milford, Delaware. In this role, you'll work alongside physical therapists to help residents enhance their mobility, regain independence, and achieve their therapeutic goals. Responsibilities: - Carry out individualized treatment plans as directed by licensed Physical Therapists. - Support residents with therapeutic exercises, functional mobility training, and other rehabilitation interventions. - Monitor and document resident progress, making note of improvements or necessary adjustments. - Communicate effectively with the healthcare team to ensure coordinated and effective care. - Educate residents and families on at-home exercises and safety practices to support recovery. - Maintain a safe and positive therapy environment. Qualifications: - Associate degree from an accredited Physical Therapist Assistant (PTA) program. - Active PTA license in Delaware or eligibility to obtain it. - Experience in a skilled nursing facility (SNF) setting is a plus. - Strong communication skills and a patient-focused approach. - Team player with a commitment to high-quality care.
