AccentCare is seeking a dedicated Admissions Registered Nurse (RN) specialized in hospice care for weekend shifts. In this vital role, the RN will be responsible for the initial assessment and admission of hospice patients, working closely with families to design personalized care plans that align with our mission to enhance patient well-being. This position requires a compassionate approach and a commitment to delivering high-quality end-of-life care. Conduct comprehensive assessments of new hospice patients. Develop and implement personalized care plans in collaboration with patients' families. Educate families and patients about hospice services and what to expect. Coordinate with healthcare providers to ensure continuity of care. Maintain accurate patient records and document care activities. Adhere to all regulatory and safety standards in hospice care.
