We are looking for a non-clinical APP to conduct outreach post discharge calls to complex patients, ensuring discharge instructions are adhered to. Go over medication reconciliation lists and confirm prescriptions are filled and picked up. Establish follow up with PCP/specialists. Nurse Practitioner and Physician Assistants Role Scope: Background • High readmission rates negatively impact patient outcomes and healthcare costs. • CMS considers readmission rates for COPD, AMI, PNA, and CVA when evaluating hospitals.
