- Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
- Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT and SLP, focused on the patient's plan of care and goals -
- Act at the direction and under the supervision of the treating Physical Therapist and in accordance with the plan of care
- Assist the treating Physical Therapist in documenting exercises in patient's chart and printing out exercises for home as directed by treating Physical Therapist
- Participate in educating the patient and the family with exercises under the direct supervision of the treating Physical Therapist, and other caregiversA few must-haves for Home Health Physical Therapists:
