The Nurse Director of Case Management will join a Northern Los Angeles Area hospital which has earned national recognition for excellence in a wide variety of specialties. This 150+ Bed medical center is known for its excellence in Cardiac Care and has been awarded as a High Performing Hospital in Heart Attack Care by U.S. News & World Report.
Reporting to the Chief Nursing Officer, the Nurse Director of Case Management will collaborate with hospital leadership and other Nursing Directors to effectively manage hospital wide care coordination. The Director of Case Management will oversee the Manager of Case Management and several Registered Nurse Case Managers.
The Nurse Director of Case Management should have over 5 years of experience working as a Case Manager in an inpatient setting as well as leadership experience. The Nurse Director will be post overseeing the organization of complex care, assessment of patient's health insurance plans and provision of necessary resources for patients being discharged. The Director of Case Management will act as an advocate for their patients and go above and beyond to help accurately assess their individual needs.
