In this role, you will work closely with a multidisciplinary team to provide direct nursing care to patients in a variety of settings, from inpatient rehabilitation to home-based care. This position offers the unique opportunity to make a real difference in the lives of individuals and their families during critical times of need. Provide high-quality nursing care to patients in rehabilitation, home health, and hospice settings. Assess, plan, implement, and evaluate nursing care plans in collaboration with the healthcare team. Educate patients and their families on health conditions and create personalized care plans. Administer treatments and medications as prescribed by physicians. Monitor patient progress and make necessary adjustments to care plans. Maintain accurate patient records and report any significant changes in patient status to appropriate team members.
