Role Description This is a full-time, hybrid role for a Care Manager based in Madison, AL. The Care Manager will be responsible for developing and implementing personalized care plans, managing client cases, and coordinating services to meet their evolving needs. Duties include overseeing care delivery, providing training to caregivers, maintaining client satisfaction, and serving as the main point of contact for families. The role is integral in ensuring top-tier care and maintaining compliance with company standards and regulations. Base: $50,000 Total comp: $75,000-100,000 Qualifications Case Management experience to coordinate care plans and oversee service delivery Proficiency in Training caregivers to ensure high-quality care standards Knowledge and expertise in Elder Care and Social Services, with a passion for working with seniors Basic understanding of Medicine and medical terminology relevant to the care industry Strong organizational, communication, and problem-solving skills Ability to work effectively in an on-site setting and maintain a client-first approach Prior experience in caregiving or healthcare leadership is a plus Bachelor’s degree in healthcare, social work, or a related field is preferred More...
